Supported Living Services (SLS) Waiver Services
Supported Living Services (SLS) support individuals who live in their own home, apartment, or with family. Through the Medicaid Waiver, SLS offers flexible, person-centered supports such as Supported Community Connections (SCC), Specialized Habilitation, and Mentorship, along with services that build independence and community engagement. Supports are provided in both home and community settings.
If you live at home or in your own apartment or house, you may receive Supported Living Services (SLS). Through the SLS Medicaid Waiver, you can access a variety of supports, including Supported Community Connections (SCC), Specialized Habilitation, or our Mentorship Program. You may also receive additional services that support you in building independence and participating in your community. These services are provided in your home and in your community.
