Choose Your Connection
We are reimagining the way Supported Community Connection services are delivered. Our approach is built around you and shaped by the belief that connection, choice, and experience are what truly enrich daily life.

Connection through experience is woven into every activity, encouraging growth through real-world participation and meaningful interaction. These are not just activities. They are pathways to connection.

Exploration Club

The Exploration Club is all about discovering new places, interests, and experiences in the community. Activities may include visiting local parks, museums, small businesses, cultural events, libraries, and neighborhood landmarks, as well as participating in seasonal or community-based activities.
Through exploration, participants expand their understanding of the world around them while developing social skills, communication, and confidence in new environments. This club emphasizes curiosity and personal interests. Participants help choose where to go and what to explore, ensuring that experiences are engaging, inclusive, and aligned with individual preferences.
ART/STEM Club

The Art/STEM Club blends creativity with hands-on learning, allowing participants to express themselves while exploring science, technology, engineering, and math concepts. Activities may include painting, sculpture, crafting, building models, simple engineering challenges, sensory experiments (like slime labs), and beginner technology projects.
This club supports problem-solving, creativity, fine motor skills, and critical thinking. Participants are encouraged to make choices about their projects, materials, and creative direction, ensuring a person-centered approach. Whether creating art or experimenting with new ideas, individuals build confidence in their abilities while engaging in meaningful, skill-building activities.
Fitness & Wellness Club

The Fitness & Wellness Club promotes physical health, emotional well-being, and self-care through engaging and accessible activities. Club activities may include group exercise, walking clubs, yoga, dance, recreational sports, mindfulness activities, and learning about healthy habits.
This club supports strength, coordination, and overall wellness while also encouraging teamwork, motivation, and positive routines. Participants choose activities based on their interests and comfort levels, ensuring inclusivity for all mobility levels. By participating in community-based fitness opportunities, individuals build confidence, social connections, and lifelong healthy habits.
Give Back Club

The Giveback Club focuses on community involvement through volunteerism and acts of kindness. Participants may write letters to community members (such as veterans or seniors), assemble care packages, support local nonprofits, participate in neighborhood clean-ups, or contribute to community events.
This club fosters a sense of purpose, empathy, and connection by allowing participants to actively contribute to their communities. Individuals help choose the causes and projects they care about, reinforcing person-centered values. Through giving back, participants build social awareness, teamwork skills, and meaningful community inclusion while recognizing the impact they can have on others.
Community Life Skills Club

The Community & Life Skills Club focuses on building independence through real-world experiences and everyday decision-making. Participants engage in activities like budgeting and money management, grocery shopping, cooking simple meals, using public transportation, and planning for future purchases or outings.
This club is designed to strengthen practical life skills while promoting confidence, self-advocacy, and independence. Participants are encouraged to make choices about what they want to learn and practice, allowing services to remain person-centered and meaningful. By practicing skills in real community settings, individuals build familiarity, comfort, and a stronger sense of belonging in their communities.
